When something is missing

Request letter for records

Some records cannot be downloaded. You have a right to a copy of your own records. Fill this in and send the letter to the provider's records office. What you type stays in your browser.

This is a plain request letter, not legal advice. Providers usually must answer within 30 days and may charge only a reasonable fee for copies.

Questions

Do I need an account?

No. There is nothing to sign up for. You add your papers, get your record, and save it to your own device.

Where is my record kept?

Only in the browser tab you are using. When you close the page it is gone. Use "Save record file" to keep a copy on your device, and "Open a saved record" to bring it back later.

Can it connect straight to my hospital portal?

No, and that is on purpose. You download your own papers and add them here, so we never need your portal password.

Is it a real medical record?

No. It is a summary for you, made by AI from the papers you add. It can make mistakes, so check it against the original papers. It is not medical advice. Ask your doctor about anything that matters for your health.

What can I add?

Visit summaries, test results, prescription lists, vaccine records, bills, insurance claims and letters. PDFs and photos both work. Password-protected PDFs need a copy saved without the password.
