The short version: OneMedRecord helps you collect and organize your own health papers. An AI reads the papers you add and builds a summary for you. It can make mistakes, so check it against your originals. It is not medical advice.

What OneMedRecord is

OneMedRecord walks you through collecting your records from the places that keep them, such as hospital portals, pharmacies, labs and insurance companies. You add your papers, and an AI builds one summary record that you can print, copy or save. It also helps you write a letter to request records you cannot download.

What it is not

We are not a health care provider, a health plan, a law firm or an insurance adviser. Nothing on this site is medical, legal, insurance or financial advice, and using it does not create a professional relationship with you. Your record here is a summary made for you. It is not an official medical record, and a doctor or hospital may not accept it in place of one. In an emergency, call 911.

AI-made content

Your record is built by AI. AI can make mistakes, including mistakes about dates, doses, test values and what a term means. Check every item against the original paper before you rely on it. Never start, stop or change a medicine or treatment because of what this site shows. Ask your doctor or pharmacist.

Using the site

Your content

You keep ownership of what you add and of the record you receive. You give us permission to process it, including by sending it to our AI service provider, only as needed to provide the service. See our Privacy page for how it is handled. Your record lives in your browser, so saving it is up to you.

Features and prices

We may add, change or remove features. If we ever charge for something, we will show the price before you pay.

No warranties

OneMedRecord is provided "as is" and "as available". We do not promise that it will be uninterrupted or error free, that it will read every paper correctly, or that it will meet your needs.

Limit of our liability

To the fullest extent the law allows, SHORE AI LLC is not liable for indirect, incidental or consequential damages, or for lost benefits, money or opportunities, arising from your use of the site. Our total liability for any claim is limited to the amount you paid us in the 12 months before the claim. Some places do not allow certain limits, so parts of this may not apply to you.

Changes and ending

We may change the site or these terms, and we will update the date above. We may suspend access for misuse. Continuing to use the site after a change means you accept it.

Governing law

These terms are governed by the laws of the State of California. Disputes will be handled in the state or federal courts located in Los Angeles County, California, unless the law gives you the right to go elsewhere.
